For your convenience we have provided a form and sample calculations to help you calculate the amount of your cheque. Please note that filling in this form will not send us any information. It can be filled out either on-line and then printed or with a pen once printed.

To calculate the amount of your cheque please do the following:
Price + shipping costs = Total bill
Canadian residents only: add GST to your total (5% x Total) = Canadian Total bill
Quebec residents only:add GST and QST to your total {7.5% x (Total x 5% )} = Quebec Total

CANPAR*	$5.95 per order, plus $1 for each additional item	3 - 9 business days eg. Tor, Mtl = 1 day, Van = 5 days

*CANPAR will not deliver to PO Boxes, please use your street address. Canpar parcels need a signature upon delivery. If no one is there to accept the package, they will leave a note and try again or send you to the nearest depot for pickup. They will however leave the package in your mailbox if you leave a note on your door the week you expect delivery asking them to leave the parcel and telling them where to leave it.
